### Documents Required for ESI Registration:
1. **Registration Form:**
– Employers need to fill out the ESI Form-01 to register under the ESI Act. This form is available on the official ESIC website.
2. **Memorandum and Articles of Association (for Companies) or Partnership Deed (for Partnerships):**
– This document is required to verify the legal structure of the business.
3. **Certificate of Incorporation (for Companies) or Registration Certificate (for Other Entities):**
– A copy of the certificate of incorporation or registration certificate issued by the relevant authorities.
4. **List of Employees:**
– A list of all employees along with their details, such as name, gender, date of joining, salary, and other relevant particulars.
5. **PAN Card of the Business:**
– A copy of the Permanent Account Number (PAN) card of the business entity.
6. **Address Proof of the Business:**
– Any document serving as proof of the business address, such as a utility bill or rental agreement.
7. **Bank Statement/Canceled Cheque:**
– A canceled cheque or a copy of the bank statement to verify the bank account details.
8. **Photographs:**
– Passport-sized photographs of the employer and all employees.
9. **Aadhaar Card of Employees:**
– Copies of the Aadhaar cards of all employees.
10. **Monthly Salary Details:**
– Information about the monthly salary paid to employees.
11. **Details of Employees’ Family Members:**
– Details of family members of employees, as required by the ESIC.
12. **Attendance Register:**
– An attendance register to keep track of daily attendance.
13. **Other Business Registration Documents (if applicable):**
– Depending on the type of business, other registration documents may be required, such as Shop and Establishment Certificate, GST Registration Certificate, etc.
### Registration Process:
1. **Visit ESIC Portal:**
– Visit the official ESIC portal (https://www.esic.in/) to register online.
2. **Fill Registration Form:**
– Complete the ESI Form-01 with accurate details.
3. **Submit Supporting Documents:**
– Upload all the required documents.
4. **Generate Challan:**
– After successful submission, the system will generate a challan for payment of the first contribution.
5. **Make Payment:**
– Pay the first ESI contribution within 15 days of the end of the calendar month in which the employee qualifies for coverage.
6. **Receive Registration Certificate:**
– After verification, the ESIC will issue the Registration Certificate.
